Five Elms Primary Care Network was formed in 2019 compromising of 5 GP practices Norheads Lane Surgery, Southborough Lane Surgery, Stock Hill Medical Centre, Summercroft Surgery and The Bromley Common Practice, Our PCN covers a population of 43,500 patients.
We focus on delivering patient-centered services that promote health and wellbeing, working collaboratively to ensure that all individuals have access to the care they need.

Five Elms PCN Enhanced Access Clinic Locations
Every second Saturday of each month, the clinic will be held at Stock Hill Medical Centre, Biggin Hill, TN16 3TJ.
For all other Saturdays in the month, the clinic will be held at Summercroft Surgery, Orpington, BR6 7AR.
